Does the bottle include a spray nozzle?

No, the bottle does not have a spray nozzle.

does this make your tire and/or garage floor slippery?

I wouldn’t apply any such material to garage floor anyway. The tires? You apply to the sides but never the tread

Is the bottle with the new branding or the old one? My concern is the expiration date. Thnx.

The bottle pictured is the new branding. I know because I have a bottle with the old branding.

can this be used on headlights and vinyl rear windows?

I would recommend a more traditional cleaner for headlights and vinyl windows.
